The request must include the manufacturer&#8217;s name and address, and contact details for a person to whom questions about the use of the Manufacturer ID may be addressed. Contact details would include telephone and fax numbers, and email address.<\/p>\n\n\n\n<p>Only one Manufacturer ID will be assigned per company. There is no charge for the use of the ID.
